New Form for Company Incorporation(SPICe+). SPICe+(Simplified Proforma for Incorporating Company Electronically Plus) PART-A. It is introduced by MCA. The portal of Ministry of corporate affairs. Using this form you can create a company easily. It is a consolidated form. Although the process is almost the same. You can do the following activities using this form:

• Apply for reservation of name.
• Apply for the DIrectors identification number. (DIN)
• Attach a copy of MOA & AOA
• Give declaration

This consolidated new form for company formation will replace old forms.

PART-A

I. Type of Company (Public/Private)

• LLPIN 
• Class of Company
• Sub-category of Company
• The main division of Industrial activity of the company
• Description of the main division
• Particulars of the proposed or approved name

PART-B

II. Structure of the Company

• Whether Articles of Association is entrenched(Yes/No).
• Number of Articles to which provisions of entrenchment shall be applicable.
• Enter details of such Articles
• Whether Company is (having a Share Capital/Not having a Share Capital)
• Enter the details of the Capital Structure of the Company
• Enter the details of members

III.  Address of the Company

• Enter the correspondence details
• Whether the address for correspondence is the address of the registered office of the company o Yes o No
• Name of the office of the Registrar of Companies in which the proposed company is to be registered

IV. Subscriber and Directors Details

• Enter Number of the first subscriber(s) to MOA and directors of the company
• Type the Particulars of the non-individual first subscriber(s) 
• Enter the Particulars of the individual first subscriber(s) (other than subscriber cum director)
• Enter the Particulars of individual first subscriber(s) cum directors
• Enter the Particulars of directors (other than first subscribers)
